This form collects only your email address and the time of your request, and stores them so we can honor your opt-out. If you booked a review, we may also hold the details you entered in our Meridian booking form: Full Name, Work Email, Personal Email, Mobile, Employer/District, Department/School, Job Title, State, Message, and the date and time of your review. If you used more than one email address (for example, a work and a personal address), submit or send each one so we can remove all of them. See our Privacy Policy for details.

We process requests promptly. A message that was already in progress when you submitted your request may still reach you for a few days.
